Scheduled to be Employed

Your wage history needs to be established. You will need to provide proof of wages when you submit your application. This can be done by attaching any of the following documents:

If you worked in this employment in 2019, please provide one of the following:

  • Previous year’s (2019) Federal Tax Return  
    • Please included the following when applicable:
      • Schedule C-Profit or Loss from Business
      • Schedule F-Profit or Loss from Farming
      • Schedule K 1- Partner's Share of Income
  • Previous year’s (2019) 1099 - Miscellaneous Income Only
  • Previous year’s (2019) W-2  
  • Final pay stub in 2019
  • If you do not have any of the above, you can provide your 2018 Federal tax returns (Note: If you are unable to provide proof of your 2019 wages, you will only be allowed the minimum PUA payment.  Once you are able to provide your proof of earnings for 2019, your claim will be redetermined. If it results in a higher amount, adjusted payments will be issued.)

If you did not work in this employment in 2019, please provide one of the following:

  • If working for an employer: Last pay stub received in 2020 
  • If self-employed: Invoice, billing, other documentation to provide proof of self-employment"

Files cannot be larger than 5MB in size and can be one of the following formats: GIF, JPG, JPEG, PNG, PDF, RTF, TXT, Excel (XLS or XLSX), or Word (DOC or DOCX).
Please note: High resolution photographs often exceed the 5 MG limit and should not be uploaded. Please choose a lower resolution setting on your camera or scanner to reduce the size of the file.

After you click the Submit button, you will receive a confirmation email. Your documents are not received until you receive this confirmation email. Please retain a copy of the confirmation email as proof of your submission.  If you do NOT receive the confirmation email, or if you are for any other reason unable to complete the document upload process, you must mail, fax or email your documents. Once a document is uploaded/submitted, it will become the property of IWD.

If you knowingly make or cause another to make a false statement or knowingly fail or cause another to fail to disclose a material fact and as a result you receive Pandemic Unemployment Assistance to which you are not entitled, you shall be subject to prosecution under section 1001 of title 18, United States Code.
