It is highly recommended you follow steps 1-5 in order to prevent any further fraud or identity theft.*

Step 1:

Complete the following form.

Iowa Workforce Development's Online Identity Theft Form

Once this form has been submitted, Iowa Workforce Development will lock the claim and the information will be sent to our Investigations Unit. An Investigator will delete the claim and only follow up if additional information is needed or if a police report needs to be filed by the victim.

If you are an employer who has submitted this form previously and are continuing to see claims filed fraudulently under your account number, please send an email to UIfraud@iwd.iowa.gov with details of the fraudulent claims that have been filed. An Investigator will review these claims and share trends that are identified with our Benefits Bureau. They will follow up with the employer only if additional information is needed.

Step 2:

Order your credit report from each of the three national credit reporting companies. These report are free after you place an initial fraud alert. You can also get a free credit report at: www.annualcreditreport.com .

Step 3:

Contact the fraud department of any related businesses if you find discrepancies on you credit report.

Step 4:

Create an identity theft affidavit online at www.identitytheft.gov

STEP 5:

Place a 7-year fraud alert on your credit file by contacting one of the three national credit reporting companies:

  • Equifax (800) 525-6285
  • Experian (888) 397-3742
  • Trans-Union (800 )680-7289
